We want our 'out of hospital' care to be the very best it can be. By providing people with the tools, skills and services they need to manage their conditions in their own communities, we will improve the quality of their lives and the lives of those who support them. The GOOHC conference will showcase the 1st year of the programme and what we will achieve at then end of the programme.
This event is opened to Oxleas staff, service users, carers and systems partners.
Delivering great 'out of hospital' care is one of our big priorities. Co-designing our services with those who use our services and their loved ones. Working in close partnership with local GPs, the voluntary sector and our local providers is helping us achieve this.
